China’s COVID-19 Cases Surge Sixfold as New Wave Gathers Pace

China reported more than 522,000 COVID-19 cases in July, a sixfold increase from June, pointing to a sharp resurgence of the virus even as authorities continue to describe the outbreak as being at a medium prevalence level.

Data released by the China Centre for Disease Control and Prevention (China CDC) on August 13 showed 522,000 confirmed cases in July, compared with 79,000 in June. The increase was 560.76 per cent month-on-month. The July tally included 487 severe cases and one death.

The China CDC said the outbreak was in a “peak plateau of fluctuation”, indicating repeated upsurges while overall transmission remained at a medium level.

But doctors and residents say the official numbers may not capture the full scale of infections. Many people with mild symptoms do not seek medical attention or undergo testing, making them unlikely to appear in official case counts.

“Besides the 522,000 confirmed cases, in reality, there are many more who haven’t been diagnosed, so the actual number is likely much higher,” a Chinese doctor said. Mild cases, he added, often recover at home after taking medication.

Another doctor said the official figures are based largely on hospital and surveillance samples and do not show how many people are actually infected nationwide.

“Nowadays, many people are not tested for mild cases, do not go to the hospital, and do not report self-test results, so the true number of infections must be far higher than the number reported,” he told local media.

Positivity Rate Crosses 20 per cent

The latest surveillance data provide another indication of the spread. China CDC data for the week of August 3-9 showed SARS-CoV-2, the virus that causes COVID-19, remained the leading pathogen among acute respiratory infections.

Its positivity rate rose to 21.2 per cent, up from 20.3 per cent the previous week.

Chinese medical experts cited by state media have warned that a positivity rate above 20 per cent could indicate a shift towards a high level of epidemic prevalence. A sustained rise over the next several weeks would therefore warrant close monitoring.

The data are particularly significant because China no longer conducts the mass testing that characterised its early pandemic response. The current surveillance system is consequently unlikely to identify every infection, especially among people with mild symptoms.

Doctors Report Rising Patient Load

Anecdotal accounts from Chinese doctors also suggest increased respiratory illness in some areas.

One doctor wrote on Chinese social media that the recent wave of COVID-19 and influenza had been severe, with dozens and sometimes hundreds of patients reporting cold and fever symptoms each day.

The doctor claimed medical staff had been instructed not to proactively recommend nucleic acid testing for COVID-19 unless patients specifically requested it.

Such accounts cannot independently establish the nationwide scale of the outbreak. But they provide additional context for official surveillance data showing a sustained rise in SARS-CoV-2 positivity.
A doctor associated with China’s disease-control system attributed the resurgence to repeated infections within the population. He also said China CDC had been conducting preparedness drills amid expectations that COVID-19 could worsen during the coming autumn and winter.

Economic Concerns Could Shape Response

China’s official COVID-19 data have faced questions over transparency since the beginning of the pandemic. The figures therefore offer useful indications of direction, but may not provide a complete picture of transmission.

The key issue now is whether the current rise is a temporary fluctuation or the start of a broader autumn-winter wave.

Authorities are unlikely to return to the sweeping lockdowns and mass testing of the zero-COVID era. Any response will have to balance public-health concerns against the economic costs of renewed restrictions.

For Beijing, that trade-off could become increasingly difficult if infections continue to climb while the economy remains a priority.

The next several weeks will be crucial. A continued rise in positivity rates, severe cases and hospital admissions would provide a clearer indication that China’s latest COVID-19 resurgence is moving beyond a limited surveillance spike.
